POSITION SUMMARY: 

As a member of Signature Addiction Medicine team the Recovery Support Navigator (RSN) will assume

responsibilities for the care coordination for patients with Substance Use Disorder(s). The Patient Navigator

works collaboratively with internal and external entities in order to remove barriers to care and ensure smooth

transitions for patients/families. The Patient Navigator works with patients to address and meet their needs by

linking the patient to appropriate Recovery Resources Harm reduction, and SUD services. They will work within

the multidisciplinary team as a non clinical support and complete department administrative duties (e.g. data

entry, phone calls, scheduling apts). They will connect with patients in the hospital and in outpatient settings

enhancing patient’s engagement in the full continuum of care. 

KEY RESPONSIBILITIES: 

1. Assist patients in referring/connecting to services and resources including addiction treatment, recovery

support, healthcare and social supports (SDoH) such as education/job trainings, housing, financial

resources, food access and transportation.

2. Assists patients in various areas to help advance their care and reduce social drivers of health by

connecting them to appropriate specialists, agencies and support systems.

3. Uses a trauma informed, person centered approach to care.

4. Maintains relationships with community entities and provides appropriate outreach when needed.

5. Assists patients with completion of required/necessary paperwork.

6. Provide administrative support in scheduling appointments with patients, provide information to patient,

obtain and verify contact information/consents, assist with care plan, etc.

7. Coordinates/removes barriers to transportation to appointments and services as needed.

8. Assists in resolving insurance issues and obtaining authorizations as needed.

9. Works with external referral sites to ensure proper referrals within and outside of Signature Healthcare.

10. Collaborates with provider and works closely with patient toward adherence to their care plan.

11. Helps patients/families solve daily problems that interfere with care.

12. Provides timely updates or changes in patient condition to the provider and other care team members.

13. Assists in data collection and data entry to monitor success for performance measures.

14. Assists in data analysis for reporting and measurement purposes.

15. Documents patient encounters in the electronic medical record in a timely manner.

16. Provides post discharge outreach to hospital patients. Contacts OBAT patient who have missed apts.

17. Maintain patient confidentiality, upholds limits of HIPAA and 42 CRF part II.

18. Conforms to Brockton Hospital and Signature Medical Group standards of performance and conduct, including those pertaining to patient rights, so that the best patient care may be provided.

19. Work setting: Split time between OBAT (176 Quincy Ave) and Hospital (680 Center St.)in Brockton (same campus-does not require commuting during day)

20. Performs other duties as assigned or necessary.

REQUIRED KNOWLEDGE & SKILLS: 

· Knowledge of addiction and addiction treatment, health care systems, and community resources.

· Ability to exercise good judgement and communicate effectively, build good relationships, show

compassion, empathy and understanding of persons with addiction, behavioral health disorders and

trauma.

· Excellent attention to detail and organizational skills.

· Ability to multi-task and work under time pressures.

· Ability to read and interpret documents accurately, such as patient charts.

· Ability to communicate well both verbally and in writing with other members of the clinical team.

· Excellent English communication skills. Additional language skills appropriate to the patient population

served (especially Spanish and/or Creole) is desirable.

· Excellent technology skills

 

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

I. Education: High School graduate or equivalent required; Bachelors degree preferred.

II. Experience (Type & Length): 1 – 2 years of successful working experience in a similar capacity. Experience in acute care setting preferred.

Prior addiction or behavioral health program experience helpful and preferred.

III. Certification/Licensure: N/A

IV. Software/Hardware: Proficiency with Microsoft Office, general knowledge of patient registration

systems and electronic medical records, basic computer skills to enter, interpret and extract medical

information from electronic systems in the healthcare environment.

V. Lived experience preferred but not required.
