Job description:
MTC Clinical Group is seeking a dedicated Recovery Case Manager to support individuals in substance use treatment through structured case management services. The Recovery Case Manager is responsible for coordinating client care, facilitating access to resources, supporting recovery goals, and collaborating with internal and external providers. This full-time position plays a vital role in client stabilization, continuity of care, and successful transition planning.
Key Responsibilities:
- Maintain a caseload of assigned clients and provide individualized case management services
- Complete comprehensive assessments, service plans, and documentation in accordance with program guidelines
- Coordinate referrals for medical, mental health, housing, employment, education, and legal services
- Develop recovery-focused treatment plans in collaboration with clinical staff and the client
- Conduct regular 1:1 sessions with clients to monitor progress, address barriers, and adjust plans as needed
- Facilitate or co-facilitate psychoeducational or recovery-based groups as assigned
- Advocate for clients in coordination with outside agencies including courts, probation/parole, DCF, shelters, and healthcare providers
- Maintain timely and accurate documentation in the Electronic Health Record (KIPU)
- Participate in multidisciplinary team meetings and case conferences
- Ensure aftercare planning is in place before discharge, including linkage to ongoing care and community supports
- Uphold program policies and model appropriate boundaries and professional behavior
- Other duties as assigned by the Program Director or Clinical Supervisor
Qualifications:
- Minimum of an Associate’s degree in Human Services, Psychology, Social Work, or a related field (Bachelor’s degree preferred)
- At least 1 year of case management or substance use treatment experience preferred
- Strong organizational and time management skills
- Knowledge of community resources and recovery supports
- Excellent communication, documentation, and interpersonal skills
- Ability to work effectively both independently and as part of a team
- Familiarity with KIPU or similar electronic health record systems is a plus
- Valid driver’s license and reliable transportation preferred
