Job Summary:   The Clinical Case Manager works as part of the Residential Services team to provide direct support to formerly homeless individuals who have transitioned into permanent housing. They operate from a low-barrier, housing first approach, to ensure residents develop skills and have access to resources they need to live increase their stabilization and maintain permanent housing The Clinical Case Manager works as part of a stabilization team to provide wrap around case management services, crisis intervention, triage and clinical consultation to stabilize residents who are currently or previously used substances, have severe mental illness, and/or have complex medical conditions. The Clinical Case Manager works collaboratively with other departments at St. Francis House to help stabilize residents in behavioral health, income, and housing.  



Essential duties / responsibilities: 

  • Welcome new residents and assist with housing stabilization.  
  • Conduct needs assessments and develops individualized service plans using a strength-based approach.  
  • Manage a caseload of 10 residents with complex needs. 
  • Collaborate with stabilization case managers to deliver wraparound services. 
  • Provide individual and group support, particularly around co-occurring mental health and substance use issues. 
  • Use best practices such as motivational interviewing and harm reduction. 
  • Maintain accurate, real-time documentation in the client database. 
  • Works with AD to provide group consultation to case managers around substance abuse and mental health of the residents  
  • Coordinates with external agencies to address complex resident needs and provide integrated support. 
  • Responds to emergency mental health situations (e.g., Section 12, BEST team calls) as necessary. 
  • Participate in supervision, team meetings, and agency-wide activities. 
  • Works collaboratively with SFH Workforce Development, Recovery Support Center, and other on-site services and community to develop, promote, and facilitate positive activities for residents living in St. Francis House housing communities.  
  • Adheres to the agency code of conduct and performs other duties as assigned.

Requirements

Required Skills & Abilities:

  • Knowledge of trauma-informed, recovery-focused, person-centered care models and de-escalation/crisis intervention techniques. 
  • Understanding of mental health disorders (DSM-V) and Best Practice approaches for working with SMI and SUD. 
  • Proven ability to engage diverse individuals, including those with CORI/SORI issues and behavioral health needs. 
  • Working knowledge of public benefits and Social Security disability programs. 
  • Reliable, proactive, independent, and responsible, with a consistent, patient, and enthusiastic approach to helping others. 
  • Ability to assess strengths and barriers, develop holistic plans for stability, and adapt client engagement strategies. 
  • Excellent verbal and written communication skills in English, strong problem-solving abilities, and experience supporting non-clinical teams with care planning. 
  • Proficient in time management and organizational skills, with a focus on attention to detail. 
  • Proficient with Microsoft Office Suite or related software. 
  • Sets and maintains clear boundaries with residents, ensuring they understand acceptable and unacceptable behavior at St. Francis House. 
  • Ability to function well in a fast-paced and stressful environment.  

 

Education & Experiences: 

  • Master's degree in social work, Counseling, or related field.  
  • LCSW (Licensed Clinical Social Worker) required; or must obtain within 3 months of hire 
  • LICSW (Licensed Independent Clinical Social Worker) license preferred.  
  • 2+ years’ experience (including internships) working with vulnerable population preferred. 
  • Lived experience of homelessness and/or recovery a plus.  
  • Experience working directly with people experiencing substance use disorder and homelessness or who have recently exited homelessness, preferred. 
  • Bilingual (Spanish), preferred.
