 In this role you will:

• Provide nurse case management for patients with OBAT-complex addictions diagnosis.
• Collaborate with team medical providers, PCPs, other providers, case management staff, patients and families to formulate a plan of care that supports best practice and guides the patient in achieving self management goals.
• Participate in development of unit goals, objectives, protocols, and procedures.  
• Utilize those skills to interact with patients/clients, team members and staff from other departments.

The successful applicant will offer:

• Graduate of an accredited nursing program.
• RN with current Mass. Licensure
• Case Management experience preferred.
• Bilingual in Spanish/Khmer/Russian beneficial
• A desire to work as a collaborative member of a multi-disciplinary team.
